1. To enter, please mail your completed form along with your name, age, address and phone
number to: SELF Magazine 1999 Health & Beauty Guide Sweepstakes, 350 Madison
Avenue, 18th Floor, New York, NY 10017. One entry per person. Entry must be received by
April 30, 1999.
2. SELF is not responsible for lost, postage due, misdirected, or late entries.
3. SELF will award one (1) winner the following prize: three (3) nights
accommodations for one (1), all meals, and three (3) treatments per day at The Greenhouse
Spa in Grand Prairie, Texas. Transportation is not included and is the responsibility of
the winner. Also includes tax and gratuity on hotel bill, except on extra services
purchased. Incidentals, telephone calls or other personal expenses are not included.
Approximate retail value is $3,000. One winner will be selected in a random drawing held
on or about May 1, 1999. All decisions of the judges are final. The winner will be
notified by phone or mail and may be required to sign and return an affidavit of
eligibility and liability/publicity release which must be returned within 21 days of date
of notification or an alternative winner may be selected. Prize is non-transferable and no
substitutions allowed except by SELF due to unavailability, in which case, a prize of
equal or greater value will be substituted. By acceptance of a prize, winner consents to
the use of his or her name and/or likeness for advertising, publicity, editorial or
promotional use without further compensation.
4. Sweepstakes participation is open to all U.S. residents, at least 18 years of age as of
the date of entry, except employees of Balance Bar Company, Clairol Inc., Ford Motor
Company, Johnson & Johnson, Orly International, Ortho-McNeil Pharmaceutical Inc., The
Condé Nast Publications Inc. and their immediate family members, their affiliates and
agents. This offer is void in Puerto Rico, outside the U.S. and wherever prohibited by law
and is subject to all federal, state and local laws and regulations. Income and other
